Default Septic Design Expiration

I was hoping for a little help from the forum, you have always come through for me.

I have a septic design for a piece of land that I own (I did not commision the design) that has a date stamp of 11/16/2004.

Questions

How long is the design good for?

Can it be extended and approx cost?

Does the septic have to be functioning by the expiration date or just be installed?

I believe state approved septic designs are good for at least 4 years. Call the person who did the design and they can tell you. Also check with the town where the design was approved for and talk to the health officer or a clerk there who should know the answer. Also the DES is very helpfull. They have copies of approved plans on file.
