I mentioned that I downloaded the form from their own website, but they assured me it was the wrong one. The link above looks like the correct form, but it is also slightly different than the "G" version they returned to me in the mail.

When I noted the time, expense, postage, and re-work involved in filling out cover letters and sending all the same stuff back and forth, the auditor noted "It's State Law."
