What, was the old girl sticking her trunk into her eye instead of her mouth ?
While still trying to figure out how an elephant might have a misfire (let alone multiple ones), let me wonder aloud...

Too Cool Man
My guess is that if the wrong temp (not brand) spark plugs were used along with a otherwise poorly tuned engine, you could dump unburned fuel down the pipe. That's death to a cat.
