When does this need to be completed by and sent in?
I have the Lakeport blue card I am sure others here on this site do to. This is my first year with it. Do they send out slips and the form or do I have to do everything?
I am unsure as I got the blue card memorial day and over the summer I killed too many braincells to remember

AC2717, Lakeport will send you everything you need including the form, or at least they have in years past. You'll just have to sign it and fill out some of the totals and mail it in.

In the past Lakeport has mailed a form together with a totaled copy of your gas receipts around the end of the year. When you receive it, sign & mail to the state. If you don't want to wait you can submit yourself by listing & mailing all your gas receipts with a self-completed form.

I thought at one time I heard that they changed the time frame for filing for the rebate and did it by quarters, gas purchased in the third quarter of the year you had to apply for the rebate in the forth quarter. Was that the case or just a rumor and if so did they change it back to yearly.:confused:
