It does state that the original MUST be sent, however as one reads down the form there is a location for "
Reason for replacement"listing the following: Change of address, Name Change,
Lost Certificate, Other. If the person is appling for a lost certificate how can they return it? That makes sense doesn't it. You must be able to use the form. Let your fingers do the walking for a few minutes and ask over the land line.
